Answer the questions below so that we can better serve you before an appointment is made to come to the office.

If “DV Shelter” is selected, the applicant must provide a verification letter from the domestic violence shelter on official shelter letterhead confirming residency. The letter must include applicant’s full name; names of all dependents who will reside in the household; all birthdates; shelter entrance date; shelter expected exit date (if applicable); case manager’s printed name and signature.  The letter must be dated within 30 days of the scheduled appointment with our agency.

You will need to have an agreement with a landlord or apartment complex prior to your intake appointment. Please have all of the required documents listed below. Please NOTE missing documentation may delay the housing process, and we will be unable to move forward until all required documents have been received in our office.
